Personal Health and Medical Record Forms

BSA guidelines require the following forms to be retained by the troop for your son:

Forms can be found here under scout forms. Blank forms are also available in the Scout hut.

CLASS 1:  Personal Health and Medical History, also includes medical information, emergency contacts, doctor's phone number, and consent for treatment. This is updated annually and is completed by parent or guardian.

CLASS 2:  Medical evaluation, updated every 36 months for participants under 40 years of age; completed by physician (or other licensed health-care practitioner as recognized by BSA). This form is required for activities over 72 hours in length involving physical activity on par with that of home or school.

CLASS 3:  Medical Evaluation, updated annually and REQUIRED ONLY for those participating in high adventure activities (including backpacking, white water rafting, etc.) AND for participants age 40 and over. Anyone participating in the upcoming October backpacking trip for first year scouts must have a class 3 form on file. This form also requires a physician's signature.

These records are REQUIRED if your son plans to attend a Summer Camp session, whether in the Atlanta Area Council or somewhere else. Also, any parent who is planning to attend summer camp, in part or in whole, must have a medical form on file.
